What bar weight should I enter?

Standard weights per foot are 0.376 lb for #3, 0.668 lb for #4, and 1.043 lb for #5 rebar. The default assumes #4, the most common size in residential slabs.

No. Lap splices, chairs, corner bars, and dowels into existing concrete add material beyond the clean grid counted here, so add them from your plan before ordering.
